Here are five questions you should never ask when collecting employee feedback, as it could present problems, according to a Small Business Trends report:

1. Are you making enough money?
2. What do you think of management?
3. What is your name?
4. Would you be more satisfied if the company provided a better benefits package?
5. Do you think the company should work to improve teamwork and workplace culture?
