Covington, La.-based Avala, a physician-led and physician-owned hospital, provided notice of a cybersecurity incident.
The hospital discovered the incident had impacted its IT systems on May 30, according to a July 28 news release from Avala.
Once it identified the breach, Avala engaged with third-party cybersecurity experts to assess, contain and remediate the incident.
Patient data that may have been exposed includes names, addresses, dates of birth, medical treatment information, health insurance information and Social Security numbers.
The hospital’s investigation found no instances of fraud or identity theft resulting from the data incident, the release said.
