- Patient factors: 39%
- Communication between patient/family and providers:28%
- Patient assessment issues: 18%
- Selection and management of therapy: 17%
- Insufficient or lack of documentation: 15%
- Communication among providers: 8%
- Failure or delay in obtaining consult or referral: 8%
