Here are four things to know:
1. The least expensive closed-network silver plan increased 9 percent from $274 to $299 per month.
2. The least expensive PPO open-access plan increased by 17 percent, from $291 to $339 per month.
3. Twenty-two states may see a decrease in silver-level PPOs in 2016. Most of Texas, New Mexico and New York will not offer silver-level PPOs, according to federal records.
4. Alternatively, Alaska, Arkansas and Wyoming are only offering PPOs.
