The initiative aims to increase collaboration between healthcare stakeholders, including hospitals, employers, physicians, nurses, patient advocates and government representatives. The initiative has two main goals:
1. Decrease the number of preventable hospital-acquired conditions by 40 percent over the next three years.
2. Reduce the number of hospital readmissions by 20 percent over the next three years by minimizing preventable complications during transitions of care.
The Obama Administration estimates achieving these goals could save around $35 billion across the healthcare system, including $10 billion savings for Medicare alone. HHS plans to use $1 billion through the Patient Protection and Affordable Care Act to support the implementation of “Partnership for Patients.”
