  • Less than 3,000 cases — 31.7 percent
  • 3,000-5,999 cases — 21.2 percent
  • More than 5,999 cases — 14.0 percent

3. $5.0-$6.9 million

  • All facilities — 33.0 percent
  • Less than 3,000 cases — 43.0 percent
  • 3,000-5,999 cases — 34.0 percent
  • More than 5,999 cases — 29.7 percent

4. $7.0-$8.9 million

  • All facilities — 29.3 percent
