The Pennsylvania Patient Safety Authority offers a free "Self-Assessment Checklist for Program Elements Associated with Preventing Wrong-Site Surgery" to help providers keep track of steps to perform to avoid wrong-site surgery.

 

The tool was published in 2010 and accompanies the Pennsylvania Patient Safety Authority report: "Insight into preventing wrong-site surgery."
