Here are five key notes:

1. Total Care is comprised of nearly 450 patient-focused care programs with approximately 118,000 medical professionals across 37 states.

2. Presently, almost 13 million BCBS members have access to care through Total Care. Total Care saves consumers an estimated $840 million annually, compared to traditional payment models.

3. Total Care emphasizes the need to spend time on prevention, holistic care and personalized care planning for patients.

4. BCBS’s program aims to facilitate strong relationships between healthcare providers and their patients.

5. The model was launched in response to the Health and Human Services’ push from a fee-for-service system to a value-based care payment model.

“Blue Cross and Blue Shield companies are leading the transformation to a better healthcare system that helps people access better, more affordable care. Our programs emphasize prevention, wellness, disease management and better coordinated care while reducing costly duplication and waste in care delivery,” said Trent Haywood, MD, chief medical officer for BCBSA.
