If a physician supplies a drug, as opposed to a patient or facility, he or she may be eligible for compensation from Medicare for the discarded portion. Medicare guidelines state drug wasted is reported in addition to the drug administered.

On the CMS-1500 claim form, physicians can list the drug units administered in box 24D. The number of wasted units is recorded as a second line item. Documentation must verify all dosages and the reason for the waste.
