Here’s what you should know.

1. Oak Street Health began employing a value-based approach to care in 2013.

2. Since implementing the changes, the health clinic saw its patient retention rate raise to 97 percent while also reducing hospitalizations by 40 percent.

3. Beginning as a single clinic, Oak Street Health expanded to 19 additional locations throughout the Midwest.

4. After seeing the facility’s success, MeridianCare entered into a value-based contract with the provider.

5. The parties did not discuss the deal’s terms.
