The Healthcare Facilities Accreditation Program and the AOA Bureau of Hospitals recently held a collaborative webcast focusing on how one hospital implemented the HFAP standards designed to improve patient safety at discharge, and this webcast is now available for replay.

Presented by Hendricks Regional Health in Indiana, the presenters discuss how a multi-disciplinary group worked to provide patients with individualized discharge notebooks, met the new HFAP hospital standards for discharge planning and have begun to reduce readmissions.
