1. Define the program. A program includes documented policy, documented processes and documented activity.
2. Build the foundation. The foundation of successful IC program will be based on each facility’s individual needs. Consider issues such as:
• Patient population
• Providers
• Personnel
• Geography
• Facility size
3. Establish assignments. Once a sound structure is in place, create specific tasks and assigned individuals or teams to accomplish these tasks.
4. Integration. Integrate the IC program with the facility peer review program and quality program. Linking these programs together enforces the overall goal of quality improvement.
