1. U, u for unit — it can be mistaken for “0,” “4” or “cc”
2. IU for international unit — it can be mistaken for “IV” or “10”
3. Q.D., QD, q.d., qd for daily — it can be mistaken for “QOD” meaning “every other day”
4. Trailing zero (X.0 mg) — the decimal point is missed
5. MS — it can mean morphine sulfate or magnesium sulfate
