Stratifying risk — Using predictive analytics to pinpoint high-risk patients

1. The report found the following segments impact outcomes:

•    Medical care — 10 percent
•    Genetics — 20 percent
•    Individual behavior — 70 percent

2. In the value-based environment, risk stratification focuses on how to keep these high-risk patients at bay from using too many medical services.

3. Healthcare providers now require real-time clinical data and patient-reported data, which are then plugged in for analytics.

4. With data analytics, providers can use risk stratification to pinpoint patient behavior, such as patients who don’t show up for appointments.

5. The report recommends providers keep an eye on high-risk patients within their risk-based contracts and utilize predictive analytics to pinpoint potential high-risk patients.
