Oregon Annual Adverse Event Summary Attributes Higher Infections to Better Reporting

The Oregon Patient Safety Commission‘s 2011 adverse event summary indicates hospitals are doing a better job of reporting events.

In 2011, 33 Oregon hospitals submitted 142 adverse event reports to the Oregon Patient Safety Commission, the highest number ever reported. In 2010, hospitals reported 125 events. According to the commission, this increase is not an indication that more adverse events are occurring, but rather that Oregon hospitals are improving their ability to identify adverse events. Not only did the quantity of reports improve, but the quality and timeliness of the reports submitted also improved.

Unintended retained foreign objects were the most frequently type of reported adverse event, followed by patient falls, medication errors and surgical errors.
