The commission collects reports from not only hospitals, but ASCs and pharmacies as well. Bethany Walmsley, executive director of the commission, told The Bulletin that “everyone is struggling with medication errors.”
Being the only state whose reporting program includes collecting information from pharmacies, less than 1 percent of eligible pharmacies submitted reports last year. Here are five key notes from the report.
1. There were 45 percent of the 624 total mistakes reported last year resulted in serious harm or death, according to the report.
2. The most common errors at ASCs were surgical errors, at 53 percent. Infections were at 12 percent and deep vein thrombosis was at 11 percent.
3. One-quarter of all errors reported last year were falls.
4. Around18 percent of all errors reported were medication errors.
5. Surgical errors or errors during invasive procedures were 15 percent.
