Here are three strategies:
1. Enlist other disciplines. Rather than focusing on a single provider type, include multiple providers in the process such as therapists who can aid in patient education.
2. Have a consistent message. Make sure all members of a patient’s care team have the same message. Therefore, use the same language to ensure every member interprets the information similarly.
3. Use your data. Update your measurement tracking and use data to enhance your processes.
