Double-Needle Method Described for Sacroiliac Joint Injections

An article in Pain Physician describes a double-needle method to improve the accuracy of sacroiliac joint injections, which can be challenging to insert in the right place each time.

 

The author, Sanjeeva Gupta, MD, said he has used the following technique on 10 patients with success:

 

  • Advance the tip of the needle into the SIJ, then check using dynamic fluoroscopy. The tip of the needle should remain within the joint line and not appear to be on the bone.

 

  • Repeat dynamic fluoroscopy to confirm that the tip of the second needle remains within the joint line.

 

  • Inject contrast dye through the needle most likely to be in the SIJ. If it is not spread satisfactorily, inject dye through the second needle.
