Here are three things to know:
1. The POEM procedure is effective in more than 90 percent of achalasia patients, but the reduction in lower esophageal sphincter pressure can lead to increased acid exposure and gastroesophageal reflux disease. Many patients require proton pump inhibitor therapy to address their GERD symptoms following the POEM procedure, but long-term PPI use is associated with negative side effects.
2. In the first case series evaluating the role of TIF post-POEM, five consecutive patients aged 18 or older underwent the procedures at a single academic center between December 2014 and June 2017. The TIF 2.0 procedure was performed with the EsophyX Z device.
3. Technical success and discontinued PPI use was achieved in all five patients. Inflammation in the three patients with esophagitis pre-procedure resolved after the TIF procedure. No adverse events were reported.
