Alabaster, Ala.-based Heart South Cardiovascular Group requested state approval to open an outpatient surgery lab.

1. The lab would provide peripheral and endovascular procedures, including iliac, femoral/popliteal and tibial/peroneal interventions.

2. It would cost $445,023.29 — excluding service and maintenance costs — to open the lab.
