Here are five takeaways:

1. The partnership will benefit more than 8,500 Connecticut residents enrolled in United HealthCare's employer-sponsored and Oxford health plans.

2. SFHCP and United HealthCare partnered years ago to focus on United HealthCare's Medicare Advantage members.

3. United HealthCare will give SFHCP added support to manage overall population health, such as technology and information that will help SFHCP-affiliated physicians take measures to enhance quality and reduce costs.

4. Through the accountable care arrangement, SFHCP can transition from a fee-for-service compensation model to a value-based approach.
