Patient safety tooklit: Project RED's new toolkit chapter

The Re-Engineered Discharge Toolkit, funded by the Agency for Healthcare Research and Quality, has added a tool to help hospitals integrate family caregivers into the discharge plan.

The tool, a new chapter in the Project RED Toolkit, targets family caregivers as a critical element in the success of the discharge plan and systematically reviews the challenges they face and the support and training they need from hospital staff. It details the process of working with family caregivers and structures it in five steps.
