Arizona's Medicaid program is investigating after the program accidentally sent personal health information to the wrong home addresses, according to azcentral, part of the USA Today network.

The key details to know:

1. A programming error caused patients' 1095-B tax forms to be mailed to incorrect home addresses.

2. Affected patients have been notified of the incident.

3. Patients' social security numbers weren't included on the forms.
