The survey included 40,044 establishments and had a response rate of 65.8 percent.

5. Tennessee
Average premium contribution and deductible: $7,900
Employee healthcare costs as a percentage of median income: 13.9 percent
6. Florida
Average premium contribution and deductible: $7,827

7. Oklahoma
