Revised CMS-1500 Form Provides ICD-9, 10 Fluidity for Code Set Transition

CMS has updated its 1500 Claim Form to prepare for the ICD-10 code set.

The new CMS-1500 form version 02/12 will replace the version 08/05 and give providers the choice of using ICD-9 or 10 diagnosis codes, which will help the transition in October 2014.

Medicare will accept the revised form as of Jan. 6, 2014, and providers have until April 1, 2014, to exclusively use the new form.
