Population Health Will Require More Accurate Illness Documentation

To be properly reimbursed under a population health-based care system, healthcare providers and coders are going to have to be able to accurately define how sick their population is, according to Health Leaders Media.

Undervaluing a population through clinical documentation could cost hospitals and practices large amounts of money.

One way to improve documentation is through adding a clinical documentation management program to get more accurate with assessing and reporting severity of illness versus observed mortality rates. However, implementation must be done carefully sure keep from negatively impacting the overall quality of care.
