1.    A provider was assisted by a resident medical student or non-physician.
2.    A technical aspect is included, such as use of equipment owned and maintained by another medical provider or facility.
3.    A procedure is more extensive than expected and requires more time or skill to complete.
4.    The same procedure was performed on both sides of the body.
