CMS will expand its Medicare Advantage Value-Based Insurance Design Model to 25 states in 2019.

Here's what you should know:

1. Medicare Advantage enrollment is at an all-time high. Approximately 33 percent of all individuals enrolled in Medicare are on a Medicare Advantage plan.

2. In 2019, CMS will expand the value-based model to an additional 15 new states. CMS will also allow Chronic Condition Special Needs Plans beneficiaries to participate in the program and allow states to propose their own system for identifying eligible enrollees.

3. According to a release, "This change will afford participants with the opportunity to include Medicare beneficiaries with different chronic conditions than those previously established by CMS ... This will provide beneficiaries across 25 states with new choices and access to customized care."

4. Ten states will spearhead the value-based model in 2018. The 15 new states will have until Jan. 26, 2018, to apply for the program.
