•    Shared savings. When accountable care organizations deliver high-quality care that results in cost-savings, its providers are rewarded for participation.
•    Patient-centered medical homes. This model provides patients with coordinated, comprehensive care. Primary care physicians are the foundation of this model.
•    Bundled payments. In this model, providers receive a single payment for all services included in an episode of care.
•    Capitation. In this model, a physician group is paid a single, set amount for services provided to a group of patients over a certain period of time, according to the report.
