The data was collected in compliance with standards, National Patient Safety Goals; Universal Protocol for Preventing Wrong Site, Wrong Procedure, Wrong Patient Surgery; and accreditation and certification requirements.

The top five most challenging requirements for ambulatory care were:

1. The organization grants initial, renewed or revised clinical privileges to individuals who are permitted by law and the organization to practice independently — 52 percent

2. The organization reduces the risk of infections associated with medical equipment, devices and supplies — 41 percent

3. The organization safely stores medications — 35 percent

4. The organization safely manages high-alert and hazardous medications — 30 percent

5. The organization offers vaccination against influenza to licensed independent practitioners and staff — 26 percent
