The organization launched a new Speak Up campaign to educate patients with an infographic and animated video and podcast as well as the Quick Safety newsletter for healthcare professionals addressing actions for improved transitions.
The “Speak Up: Avoid a return trip to the hospital” resources are free and available on The Joint Commission’s website. The pre-discharge information includes:
• Facts about treatment and diagnosis
• Medication
• Follow-up care
• Information on where to receive help
There is also post-discharge information to explain what patients might experience depending on their condition and where they’re discharged.
The Quick Safe newsletter focuses on improving transitions and involving patients and families in the process. Topics in the newsletter include:
• Enabling families to visit around the clock
• Physician and interdisciplinary rounds at the patient’s bedside
• Change of nurse shift reports delivered at the patient’s bedside
• Patient-centered discharge planning
• EHRs patients can access and edit
