The Joint Commission introduces SAFER matrix to identify ambulatory risk, improve patient care

The Joint Commission has integrated its Survey Analysis for Evaluating Risk Matrix into its accreditation program, according to an Ambulatory Buzz post.

Here’s what you should know.

1. SAFER is a key component of The Joint Commission’s REFRESH initiative. It applies to all accreditation and certification programs.

2. REFRESH is The Joint Commission’s attempt to modernize aspects of its surveying process.

3. The matrix is replacing The Joint Commission’s former scoring methodology.

4. SAFER allows surveyors to quantify how every requirement for improvement could harm a patient, staff member or visitor based on the deficiency.
