Here’s what you should know:
1. California-specific ambulatory care organizations, hospitals and critical access hospitals that offer primary care services can now fulfill Covered California’s incentives by earning The Joint Commission’s PCMH Certification.
2. More than 20 practices are already qualified to participate through the PCMH certification earned through The Joint Commission’s Ambulatory Care Program.
3. The Joint Commission’s Primary Care Medical Home Certification is a three-year award, bestowed after an onsite accreditation survey process.
