3. Ambulatory care: 4 percent
4. Psych unit in general hospital: 5 percent
5. Emergency department: 6 percent
6. Behavioral health facility: 4 percent

8. Long term care facility: 1 percent
9. Other, including disease specific care, hospice, diagnostic imaging: 1 percent
