AAAHC-recommended elements for quality initiative reports include:
1. Purpose — State the problem and its importance to the organization.
2. Performance goal — State the initiative’s performance goal and the rational behind the goal’s selection.
3. Description of the data — Note what kinds of data were collected, why they were collected and what the data was used for.
4. Evidence of data collection — Record who collected the data, data sources, samples, sample descriptions and length of data collection, and include data collection forms.
5. Analyzed data — Include any charts/graphs of data. Comment on findings extracted from the data.
6. Current performance compared to performance goal
7. Corrective action — List corrective actions needed to take current performance to the performance goal.
8. Re-measuring — Comment on when re-measuring occurred relative to corrective action, and state the relationship between that outcome and the performance goal.
9. Additional corrective action and re-measuring — Again, include designated re-measuring times, additional corrective actions and results compared to the performance goal.
10. Communication of Findings — Note to whom the findings of the intervention were communicated, how they were communicated and where records of that communication are available.
