2. Dr. Beacom completed his internship and residency programs at the University of Illinois in Chicago.

3. Dr. Beacom also has affiliations Swedish Covenant Hospital in Chicago and Maywood, Ill.-based Loyola Gottlieb Memorial Hospital.

4. Dr. Molnar completed his internship and residency programs through the Medical College of Wisconsin in Milwaukee and is affiliated with Saint Elizabeth Regional Medical Center in Lincoln, Neb.
