1. 10 percent to 19.9 percent of revenue: 12 percent of ASCs
2. 20 percent to 29.9 percent of revenue: 65 percent of ASCs
3. 30 percent to 39.9 percent of revenue: 24 percent of ASCs

Occupancy

4. 0 percent to 9.9 percent of revenue: 50 percent of ASCs
5. 10 percent to 1.99 percent of revenue: 38 percent of ASCs
6. 20 percent to 29.9 percent of revenue: 13 percent of ASCs

Drugs, medical supplies and implantables

7. 10 percent to 19.9 percent of revenue: 19 percent of ASCs
8. 20 percent to 29.9 percent of revenue: 62 percent of ASCs
