Patient Safety Tool: Readmission Diagnostic Worksheet

The Institute for Healthcare Improvement in Cambridge, Mass., offers a free, downloadable readmission diagnostic worksheet, designed to help hospitals identify opportunities for improvement based on reviews of the five most recent rehospitalizations.

The diagnostic worksheet includes chart reviews and interviews with readmitted patients and their family. This tool was originally developed as part of the STate Action on Avoidable Rehospitalizations initiative.
