Compensating Physicians in Surgery Center Administrative Roles: Q&A With Ben Ulrich of VMG Health

Q: I’m a physician who is the sole owner of my ambulatory surgery center. I am serving as the administrator and medical director as well. How should I determine what I should be paid for filling those positions and how should I be paid?
 
Ben Ulrich: Fair market value compensation for physicians serving in administrative roles can vary by many factors. Understanding whether a set of services requires a physician, and more specifically, a physician’s particular medical specialty, is paramount in determining appropriate administrative compensation.

For positions like medical directors, which typically require physicians and particular medical specializations, there is a wide variety of clinical and medical director market compensation survey data available. Alternatively, surgery center administrator positions could be provided by either physicians or non-physicians. From a regulatory perspective, compensation paid should reflect what other administrators are paid in the marketplace.

 

That being said, a physician who has extensive experience in running an ASC should receive administrator salary towards the upper end of market survey data for these services. To ensure compliant and reasonable compensation rates, we would suggest having a full FMV appraisal conducted.
