Patient Safety Tool: Serious Event Response Toolkit

The Institute for Healthcare Improvement offers a free, downloadable toolkit to help healthcare organizations prepare for a response to a serious adverse event.

The toolkit was developed by Christiana Care Health System, based in Wilmington, Del. It includes a process flow chart for communicating serious adverse events, checklists for mitigating the harm after an adverse event and a guideline for debriefing after an adverse event.
