The Independent Patient Safety Investigation Service and NHS organizations will look into how investigations are handled regarding patient safety incidents, according to Good Health Suite.
Here are four notes:
1. IPSIS will look into incidents in 2016.
2. An expert advisory group is deciding what they should investigate and which people and resources will be essential to the investigations.
3. Mike Durkin, MD, director of patient safety at NHS England, heads the expert advisory group.
4. Dr. Durkin believes system errors and not human errors pose the most problems in healthcare.
