Manual Anthony Puig, who did business as Puig Medical Services, operated the Alton, Texas-based La Hacienda Family Clinic. As a physician assistant, Mr. Puig was required by state law to have a licensed physician supervising his work and delegating responsibilities to him. Mr. Puig admitted that he conspired with others to submit claims to Texas Medicaid by fraudulently using the Medicaid provider number of R.J.P., a physician who, for some time before 2005 until his death, was physically and mentally unable to provide medical services or delegate authority to Mr. Puig to provide medical services.
Mr. Puig also admitted that he submitted a written notice to the Texas State Board of Physician Assistant Examiners that fraudulently claimed R.J.P. would supervise his attempts to provide medical services at La Hacienda Family Clinic. He used R.J.P.’s provider number to submit over 6,000 claims to Medicaid for which Medicaid paid approximately $173,830.56.
Sentencing for Mr. Puig is scheduled for Oct. 26.
