Texas Hospital Implements “Care Transitions” Initiative to Reduce Hospital Readmission Rates

Temple, Texas-based Scott & White Healthcare has implemented a “care transitions” initiative designed to keeping readmission rates low, according to a report by U.S. News and World Report.

Healthcare reform is clamping down on the rate of hospital readmissions and penalizing facilities by not reimbursement them for readmissions within 30 days of discharge. Scott & White Healthcare is implementing a new strategy in light of the reform by assigning trained social workers to serve as “transition coaches” to bring more care to the homes of patients upon their release from the hospital.

Transition coaches will visit patients’ homes, review medications and advise patients on how to call for extra assistance when it is needed. Coaches will also make phone calls throughout the week to check up on patients.
