Individualized care plans that take into account patients’ specific needs are more likely to improve health outcomes than plans that do not address patient-specific factors, according to a study in Annals of Internal Medicine.

Researchers studied patient-centered decision-making, which they define as “the process of identifying clinically relevant, patient-specific circumstances and behaviors to formulate a contextually appropriate care plan.”
