The Safety Attitudes Questionnaire includes five points:
• Is it easy for personnel to ask questions when there is something that they do not
understand?
• I have the support I need from other personnel to care for patients.
• Important issues are well communicated at shift changes.
• Briefings are common in this clinical area.
• My suggestions about safety would be acted upon if I expressed them to management.
Respondents have the option to respond over a range of frequencies, from “always” to “never.”
