The ACO Implementation Collaborative will help health systems ready for ACOs go through implementation. The ACO Readiness Collaborative will help systems that still need to make some preparations before launching ACOs.

ACOs involve bringing together a wide range of providers to deliver comprehensive care for patients to educe healthcare expenditures. Under the health reform law, CMS will share the savings with ACOs.

The collaboratives will work on the following components of ACOs:

  • Designing the ACO from a patient’s perspective.
  • Creating health homes to deliver primary care and manage health and wellness.
  • Unifying primary, specialty and hospital care to reward coordination, efficiency and productivity.
  • Tightly integrating relationships with specialists, ancillary providers and hospitals.
  • Creating reimbursement models through provider-payer partnerships.
  • Extending healthcare IT across provider networks, such as health information exchanges.
