The tool, entitled Your Discharge Planning Checklist, lists action items patients should take in order to ensure a smooth and successful discharge. These items include asking healthcare providers where a patient will receive care after discharge and the name and phone number of the caregiver helping the patient after discharge.
The checklist also includes a drug list, intended to help patients understand how and when to take their medicine, and list of scheduled appointments.
