According to the AHRQ proposal, many adverse events go unreported because most reporting systems do not accept reports from patients and their families. AHRQ proposed a consumer reporting system could help produce a more thorough and accurate understanding of how adverse events occur and how to prevent them. The system would include a safety event intake form and healthcare provider follow-up.
