A staff nurse discovered and reported the issue, and any providers involved in cleaning this scope were immediately educated on proper cleaning. In addition, a third party consultant reviewed the hospital’s scope cleaning process via direct observation and concluded the current process meets national quality and safety standards, according to the report.

The hospital has contacted the 10 patients who had procedures with this scope during the specified time. The patients will be tested for infections.
“I know I speak for all caregivers at Mission Hospital that patient safety is our number one priority. We are deeply saddened that this has occurred. While we have taken necessary steps to address this issue, we will continue to accelerate our plans and efforts to insure the highest possible safety,” said Jill Hoggard-Green, PhD, RN, president and COO of Mission Hospital, in the report.
