The PCPCC, created in late 2006, is a coalition of more than 700 major employers, consumer groups, organizations representing primary care physicians and other stakeholders who have joined to advance the patient-centered medical home, an approach to providing comprehensive primary care for children, youth and adults.

“We are proud to be the first accrediting organization to join the Collaborative, at the Executive Committee level” said Michael J. Zarski, JD, and CEO of HFAP, according to a news release. “Supporting the medical home concept ties into our major emphasis on continuity of care quality initiatives.”
